There are a number of forms involved with purchasing and maintaining a grave.

Notices of interment (burial application form)

 

A notice of interment form is required for all burials, whether the burial is for a full body or cremated remains. These are kept as part of the official burial records.

If the registered owner of the grave is the deceased, then the form needs to be signed by the person arranging the funeral. If the burial is for someone other than the registered owner, then the registered owner(s) will have to sign the form.
